Mission of Hope – Australia specialises in providing the care and support you need while transitioning. We aim to create more awareness about your funded support and how you can make the most of it. We help you strengthen your connections and improve your community participation. Our goal is to assist you in building essential life skills to lead an independent life. Our services are flexibly structured to cater to your individual needs.
Support Coordination helps you discuss your goals with the NDIS service provider. You can plan your budget and discuss the effectiveness of the support you are receiving. With support coordination, we will help you decide and navigate your NDIS plan.
Level I Support: We help you understand your NDIS plan and connect with the NDIS community. We aim to improve your skills and confidence to independently manage your plan and get the most out of it.
Level II Support: We will assist you in curating a special mix of services needed to improve your capabilities and lead an independent life. We will help you manage tasks and maintain relationships while working towards your goals.
